Basic Needs Assistance Form
This form will be assessed to provide individual resource suggestions and options to meet with supportive staff for a Basic Needs Consultation.

If you have any questions about CalFresh eligibility or need assistance with your CalFresh application, please use this form.
Personal Hygiene Form
Do you need personal hygiene products? Fill out this Google Form to request items you may need, and to set up a pick-up time to retrieve your items from The Hub Basic Needs Center.

We strive to increase equitable access to food, housing, and financial wellness resources for all students by breaking down barriers and engaging in transformative dialogues surrounding students’ basic needs so they can focus on their academic success contributing to holistic well-being.
